CURRENT SCHOOL YEAR REGISTRATION

 

You will need to come and register in the Guidance Department.  If you wish to make the process easier, please print off the forms, listed on the right, and fill them out completely.   

You will also need to bring the following items with you to register your student.   

 

Two (2) types of Proof of Residency for St. Charles High School 

(Current Rental Contract, Current Mortgage Statement or Current Real Estate Tax Receipt AND 

one of the following - Unpaid Utility Bill, Current Bank Statement, Current Personal Property Tax receipt or Current Voter Registration Card)

 

• Birth Certificate 

• Current Immunizations 

• Parent/Guardian Photo ID 

• Divorce/Custody/Guardianship Paperwork 

• IEP and most recent Evaluation  (If applicable) 

 

If transferring mid year, please bring in a withdrawal form from the student's previous school which should include transfer grades for current classes.  

 

If your student has an IEP, please bring a current copy and evaluation with you.
